FROM THE REGISTRAR

PROGRESS REPORTS AND REPORT CARDS
Six and twelve week Progress Reports are handed to the students during their 4th period class. Check school calendar for dates. Semester Report Cards are mailed home.

GRADE CHANGES
A request for the first semester grade change should be made within two weeks after the reports cards are received. A request for a second semester grade change should be made within two weeks after the beginning of the next school year. Grade change forms are available from the Registrar. It is the student’s responsibility to obtain this form and give it to the instructor. It is the instructor’s responsibility to return the completed form to the Registrar.

TRANSCRIPT REQUESTS
A student can request a transcript for student information, college applications, and scholarship applications. The student can sign-up for a transcript on the request sheet, at the Registrar’s office, in the morning and pick it up at the end of the day. All requests are handled on a daily basis. While enrolled at SLVHS, there is no fee for transcript services; however, after graduation there is a $5.00 fee.

TRANSFERRING SCHOOLS
When transferring schools, the student must obtain a Checkout Sheet from the Registrar. This sheet needs to be signed by a parent, by each instructor, by the bookroom clerk and the librarian. In order to be cleared by the bookroom and library, the student must return all books and pay all sports and class or library fees. Upon receipt of the Checkout Sheet, the Registrar will give the student a copy of the records required for enrollment at another school (immunization records, transfer grades and transcript).

TEXTBOOKS
Textbooks are provided free of charge on a loan basis. Students assume the responsibility for all books issued to them and should return the books in good condition. ALL BOOKS MUST BE ACCOUNTED FOR PRIOR TO GRADUATION.

During the course of the year, bills will be sent to students in their classroom as a reminder to pay for lost books and overdue fines. Any bills over $30 will be mailed home and due upon receipt.

Lost or damaged book fees must be paid before a new book is issued. All outstanding fees must be cleared before students can pick up their schedules in August, purchase a prom bid, pick up their yearbook, or receive their diploma. Students can borrow textbooks from the library on a one period basis until the lost book is paid for. If the book is found, the computer automatically issues a refund notice. A check will be issued by the District Office and delivered to the student in class.
